From 8 March 2027, the End-of-Life Pathway will offer up to 24 weeks of support for people who wish to remain at home.

Services Australia has confirmed that the End-of-Life Pathway for aged care will be extended from 8 March 2027. Under the change, care recipients who are still living at home once the first 12 weeks are up will automatically continue receiving support for a further 12 weeks, taking the full pathway to 24 weeks.

End-of-Life Pathway funding

The extension comes with additional funding of about $25,000 for people who remain in their own home after the first 12 weeks. There will be no need for a reassessment, and the eligibility criteria are not changing.

How the End-of-Life Pathway works today

At present, eligible care recipients can use the pathway for 12 weeks. Where funding is left over, support can continue for up to 16 weeks. From March 2027, people who stay at home will automatically move into a second 12-week period with fresh funding, rather than relying on whatever is left from the first.

Provider claims

Services Australia has confirmed that providers delivering care under the pathway can continue to claim services under existing arrangements.
